The following forms are due prior to the first day of class with the exception of the medical statement which you have 30 days to complete with physician's signature. Child Medical Statement (current for 13 months after date of exam) Child Enrollment and Health Information (3 pages) Departure List and Family Password (available from preschool upon enrollment) |
